過敏氣喘,細胞免疫,基因治療、保健食品開發:

        從1970年代以來,許多報告都指出,全世界氣喘病的罹患率持續在增加,病情轉趨嚴重,住院率和死亡率也隨著增高,台灣地區也不例外。在氣喘病人身上可觀察到幾項特徵:包括血液中的IgE增加,體內第二型T輔助細胞(Th2)數目增加,氣管聚集大量的嗜伊紅性白血球細胞(eosinophilia)以及發炎媒介物的產生。過去對氣喘疾病的治療大部分偏重於抑制氣管的發炎現象和舒緩氣管的收縮程度,而吸入性類固醇的使用為目前對於氣喘症狀控制和肺功能的改善最有效的方法;但是類固醇的使用只能抑制發炎現象,對氣喘仍無法根治,而且對於人體仍有潛在的副作用,所以病人除了儘量避免接觸過敏原外,要徹底治療氣喘疾病,目前治療方法為減敏療法。減敏治療必須持續幾年的治療,此種耗時並且需要大量純化過敏原的治療方式並不是最理想的治療方法。基於此,其病因的探討和研發過敏氣喘新治療的方法則刻不容緩。

在大家致力研發氣喘疾病的治療新方法中,有兩種策略是被認為最具有發展潛力的:

(1) 發展出能誘發氣喘病人體內第一型T輔助細胞(Th1)免疫反應的疫苗,用來抑制過敏性第二型T輔助細胞(Th2)的免疫作用。而目前已知細胞激素IL-12, IL-18, IL-27以及IFN-g皆可用於引發Th1細胞的免疫反應。

(2) 發展出能誘發氣喘病人體內調節性T細胞(Treg)免疫反應的疫苗,用來抑制過敏性第二型T輔助細胞(Th2)的免疫反應。而目前則認為細胞激素IL-10可做為佐劑,用來幫助病人體內調節性T細胞的生成,進而抑制Th2細胞的免疫作用。

敝研究室主要研究方向是探討過敏疾病的治病機轉,並利用此依研究結果來研發近一步的免疫療法。多年前我們研究室就已經開始將IL-12、IL-10、益生菌及樟芝保健食品的純化物應用到過敏氣喘動物的治療與預防,在對於預防或減緩氣管發炎的小鼠上皆獲得不錯的成績。目前,則致力於開發更有效的治療產品與治療方式,我們的實驗室將使用各種(1)表現Notch配位子基因的載體;(2)類鐸受器(Toll-like receptor)促效劑;(3)益生菌;(4) 西洋蔘達瑪烷苷;(5)納豆發酵純化物…等;評估這些物質在氣管發炎的動物模式下,是否能建立一個更有效的治療的新方法。
